Each student is responsible for filling out their own certification application form and sending it, along with the exam fee, directly to ASQ headquarters. The Body of Knowledge and the exam application form are included in the Certification brochure which is available at no cost from ASQ by calling 800-248-1946.

  • Deadline: Two weeks prior to start of the class or contact Al Scorza

Recertification

Re-certification is an important aspect of your ASQ membership. Every three years your CQ Mgr, CQE, CQA, CRE, CSQE, and Six Sigma Black Belt certifications must be re-qualified to continue being recognized. You have the choice between retaking the particular exam or accumulating re-certification units (RU's) by working in the Quality field and attending monthly chapter meetings, education classes and seminars to name just a few.

If your certification is coming due, please forward your re-certification journals along with the appropriate processing fee to the Recertification Chair for review. Checks should be made out to ASQ. Also, if you have multiple certifications, determine if it would be beneficial for you to synchronize them together. Please send copies of your packages, as these will not be returned to you. We look forward to helping you in this process.
